POSITION SUMMARY:
  • Reporting directly to the RN Clinical Manager of Bridge Services with clinical oversight by Vice President of Care and Quality (LCSW).  The Palliative Care Licensed Clinical Social Worker will ensure a high quality of psychosocial support is being provided consistent with the mission of the hospital. The Palliative Care Licensed Clinical Social Worker will assist in the coordination of care through the use of both internal programs as well as identified community stakeholders to serve terminally ill, chronic and complex care patients through education, care management, quality improvement, and support programs. This position requires full understanding of the psychosocial implications that result from end of life decisions, palliative care, care transitions, and chronic disease management. The Palliative Care Licensed Clinical Social Worker will contribute to the training and education of others, developing clinical practice across all aspects of  care transitions, complex care management and palliative care. He / She will be expected to research and promote the use of evidence-based practice within the multidisciplinary team.   

POSITION QUALIFICATIONS:
  • Must have strong verbal communication skills required to deal with interpersonal situations in a professional manner.
  • Must have the ability to identify areas in which the population served would benefit from support groups, formulate material, and effectively facilitate group work.
  • Must have the ability to speak effectively before groups of customers or employees of organization.
  • Bilingual English/Spanish is a plus 

Education/License/Certification:
  • Must have a Master's in Social Work from an accredited university
  • Must obtain LCSW within 18 months of hire
