Social Work Assistant

*Shift: 8:00A - 4:30P

POSITION SUMMARY:
  • Reporting to the Director of Care Continuum and under the supervision of the Licensed Clinical Social Worker (LCSW), the Social Services Assistant works as a member of the interdisciplinary team to help patients and families understand and cope  with the  emotional and social problems that affect their health status, and to find and use the  resources needed for a smooth transition from hospital to home.

POSITION QUALIFICATIONS:

Education/License/Certification:
  • Must have a Bachelor’s Degree in Social Work, Psychology, Counseling, or related field, with evidence of work experience in health care or similar setting.

Experience/Skills:
  • Must have experience in health care or similar setting.
