Population Health Scheduler/Clerk (CNA)
To perform this job successfully, an individual should be a high school graduate or equivalent. Must be able to take vital signs and chart them accordingly. Basic knowledge of medical terminology is a necessity. Experience in Medical Office reception or hospital CNA/Clerk role preferred.
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence if required. Advanced communication and organizational skills with excellent phone etiquette. Bilingual in English/Spanish preferred. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. To perform this job successfully, the individual should have knowledge of office machines and computer data entry and type 35 wpm accurately.
Under direction of the RN Manager of Population Health, the Population Health Scheduler/Clerk performs a wide range of routine clerical duties related to the coordination of care for the Population Health patients. The majority of the patients will require flexible scheduling and excellent follow through. This position involves communicating with patients both over the phone and in person, taking of vital signs, and documentation of the data.
Must be able to work normal/scheduled working hours which may include Holidays, call-backs, weeknights, weekends, and on-call in the Director's absence. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours.
